Magnesium has become one of the fastest-growing supplement categories in the United States, with sales roughly tripling between 2017 and 2023 as social media pushed separate forms for sleep, anxiety, digestion and cramps. The ingredient itself matters: magnesium is a cofactor in more than 300 enzyme systems, including muscle and nerve function, blood glucose control and blood pressure regulation, per the National Institutes of Health Office of Dietary Supplements. But the form-specific promises on labels are ahead of the trials, and dosing errors are common enough that the element deserves more scrutiny than it gets.

How much magnesium do adults actually need?
The Recommended Dietary Allowance is 400–420 mg per day for men and 310–320 mg for women, rising to 350–360 mg in pregnancy, with an upper limit of 350 mg per day from supplements and medications only — not food. Roughly half of Americans consume less than the estimated average requirement from food, per national survey data, which is the kernel of truth behind the marketing. Good food sources include pumpkin seeds (about 156 mg per ounce), almonds, spinach, black beans and fortified breakfast cereals, so diet alone closes most gaps for people who eat these regularly.
Do the forms really differ?
Yes — but mostly in absorption and osmotic effect, not in exotic targeted functions:
- Oxide. The cheapest and most common form, roughly 4 percent elemental magnesium by bioavailability studies. Poorly absorbed, which is exactly why it works as a laxative and antacid (milk of magnesia is magnesium hydroxide, a close relative).
- Citrate. Better absorbed than oxide and reliably osmotic — it pulls water into the intestine. Clinically used for constipation and bowel prep. The marketing label "for energy" oversells; the practical effect is mostly on the gut.
- Glycinate (bisglycinate). Magnesium bound to the amino acid glycine. Well absorbed and gentler on the stomach than oxide, which makes it a reasonable choice for people who need ongoing supplementation. Claims that glycinate treats anxiety or insomnia lean on glycine's calming reputation, not on solid magnesium-specific trials.
- Malate, taurate, threonate. Marketed for muscles, heart and brain respectively. A small 2024–2025 wave of preliminary animal work on magnesium threonate and cognition has not yet translated into convincing human trial evidence.
- Chloride and sulfate. Found in topical sprays and Epsom salts. Transdermal magnesium absorption is poorly demonstrated; a warm Epsom bath may relax muscles, but the mechanism is warm water, not measurable magnesium uptake.

What do trials actually support?
The best-documented uses are clinical and narrow. Magnesium sulfate is a first-line hospital treatment for severe pre-eclampsia and for torsades de pointes, a dangerous heart rhythm. Oral supplementation shows modest blood-pressure reductions in meta-analyses of people with existing hypertension — small effects, typically a few mmHg. Evidence for migraine prevention is mixed but led the American Migraine Foundation to list magnesium among options worth discussing; doses studied run 400–600 mg daily of oxide or citrate. For leg cramps in pregnancy, trial evidence is weak. For sleep and anxiety — the two biggest marketing drivers — randomized trial data remain thin, small, and inconsistent, and the 2025 social-media boom has outrun the science.
What are the real risks?
Excess oral magnesium that the gut cannot absorb simply draws water and causes diarrhea, cramping and nausea — unpleasant and self-limiting for most people. The serious risk sits with kidneys: hypermagnesemia from chronic high dosing in renal impairment can cause low blood pressure, confusion, slow reflexes, breathing difficulty and cardiac arrest. The NIH Office of Dietary Supplements also documents interactions worth knowing: magnesium can reduce absorption of certain antibiotics (fluoroquinolones and tetracyclines) and bisphosphonates like alendronate, so timing doses several hours apart matters, and it can amplify the effect of blood pressure medications.
What actually depletes magnesium?
Beyond low dietary intake, several common situations raise the risk: long-term use of proton pump inhibitors such as omeprazole (a magnesium-wasting effect the FDA warned about in 2011), diuretics like furosemide and hydrochlorothiazide, type 2 diabetes with poor glucose control (which increases urinary losses), chronic diarrhea, and alcohol use disorder. Older adults absorb less and excrete more. If any of these apply, that is a stronger argument for a supplement than a sleep-hack video, and a reason to tell your clinician, because these same medications may need dose timing adjustments.
How is magnesium status tested?
The routine serum magnesium test is inexpensive but imperfect: only about one percent of total body magnesium circulates in blood, so a normal serum value does not fully rule out depletion inside cells. Research settings sometimes use an intravenous loading test or measure red-cell magnesium, but neither is standard in primary care. In practice clinicians decide by combining symptoms, dietary history, medications and the serum value. Ask what your result means in context rather than treating a single number as definitive.
How should a label be read?
Check the elemental magnesium amount per serving, not the compound weight — a "1,000 mg magnesium citrate" capsule may deliver only about 160 mg of elemental magnesium. Compare that to the 350 mg supplement upper limit and to total intake if you also use antacids or laxatives containing magnesium. Products certified by USP or NSF have been third-party tested for content accuracy, which matters in a category where assays have historically found label inconsistencies.
What to do next
If you suspect low magnesium — fatigue, muscle twitching, loss of appetite — ask about a blood test rather than guessing, keeping in mind that serum levels only reflect about one percent of body stores, so clinicians also weigh diet and medications. Start with food sources, choose citrate or glycinate if a supplement is warranted, stay under 350 mg of supplemental elemental magnesium daily unless directed otherwise, and expect citrate's bowel effect. If constipation is your actual complaint, that is not a side effect to avoid — it is the active ingredient.
